Undergraduate
- Flagging Petition to the Faculty
- Singly enrolled SOE students who take the same class more than once and wish to flag the lowest grade
- Transfer Credit Petition
- Health and Exercise science, physical education and selected studies in education students only. Inclusive elementary and special education students use a different form sent by Marie Sarno. Students wishing to take a class at another institution will complete this form prior to taking the course to obtain written approval.
Masters
- M.S. Program of Study
- MS students in SOE. Please note this new version requires a student signature.
- Request for Master Exam or Portfolio Presentation
- This form is competed by a student if specified by their faculty adviser. It is used when faculty request the office of academic and student services prepare the Request for Exam form for the faculty adviser to complete. (Note: not all SOE departments use this form).
- Results of Exam,Thesis, or Portfolio Form
- For documenting the results of the exam, thesis or portfolio.
Certificate of Advanced Study
- C.A.S. Program of Study Form
- CAS students in SOE
- Request for C.A.S. Qualifying Exam
- This review and/or examination timeline is determined by the faculty/program adviser. The application should be submitted to the offic eof academic and studetn services no later than 2 weeks prior to the exam.
Ph. D.
- All But Dissertation (ABD) Status Form
- To certify that a student has officially attained the status of ABD by virtue of having completed departmental requirements.
- Request for Doctoral or C.A.S. Qualifying Exam
- This review and/or examination usually occurs at the end of a students secondsemester of full time study or upon completion of the 45th credit hour of course work for doctoral students. Students must file this application at least two weeks prior to taking the exam.

- Sample proposal cover page.
- Doctoral Checklist
- Internal document between the staff in the Office of Academic and Student Services and the doctoral student. It is also available in the Ph.D. Orange Book.
- Formal Doctoral Program Plan
- For completion after the preliminary review, the formal program plan is submitted for approval.
- Informal Doctoral Program Plan
- For completion during the first semester of study as a guide to do some long term planning for completing the Ph.D.
- Intent to Defend Form
- Ph.D. Apprenticeship Registration Form
- To obtain advanced agreement with the faculty program advisor and faculty research apprenticeship sponsor regarding the research apprenticeship.
- Request for Examination
- Research Apprenticeship Approval
- To provide proof of completion.
