Forms for All Students

Flagging Petition to the Faculty Student, Senior Assistant Dean When a student has repeated a course and requests a flag of the original grade. Both grades will remain on transcript, but only the highest grade will be counted towards the cumulative grade point average.
Proposal for Independent Study Course
Internship Proposal Agreement
Student, Faculty Sponsor, Department Chair, Academic Advisor, Senior Assistant Dean Student and faculty sponsor must completely fill form out to ensure accuracy for each bullet point.
Withdrawal Petition Student, Instructor, Academic Advisor, Department Chair, Senior Assistant Dean After academic drop deadline, but before the withdrawal deadline, stduents may request to withdraw from a class. Classes from which students withdraw remain on the transcript record with the grading symbol WD.
Leave of Absence: leave for semester or withdraw from the University (paper form only; available from Academic Services) Student, Academic Advisor or Senior Assistant Dean Completed online; contact your advisor.
Suspended students: Discuss plans with your advisor, and then send readmit letter to Academic Services.
Medical leave students: Also contact the Office of Student Assistance.
Judicial student: Must be cleared through the Office of Student Rights and Responsibilities.
Application for Readmission/Termination of Leave of Absence (paper form only; available from Academic Services) Student, Accepting Dual/Combined College (if applicable), Senior Assistant Dean This request must be made with your academic advisor via email.
Request for Incomplete Grade Student, Instructor, Department Chair, Senior Assistant Dean An incomplete grade may be granted when exceptional circumstances prevent a student from completing course requirements within the normal time limits.
Application for Undergraduate Intra-University Transfer (IUT) Student, Senior Assistant Dean Students should meet with Academic Services for information and forms regarding their new major.
Declaration of Minor Student, Faculty Advisor, Minor Coordinator, Senior Assistant Dean Declare minor, request additional minor, or drop a previously declared minor.
Petition to Walk (Early Participation in Commencement) Student, Academic Advisor, Senior Assistant Dean Contact your academic advisor for details.
Release of Academic Information – FERPA Authorization Student, Senior Assistant Dean FERPA protects the privacy of education records and the sharing of information, establishes the rights of students to inspect and review their education records, and provides guidelines for the correction of inaccurate and misleading data through formal and informal hearings.

To ensure appropriateness of the course, approval should be obtained BEFORE taking it. Attach course description and submit before course is taken.
Registration Advising Form Student, Academic or Faculty Advisor Providing main course and alternative course selections with advisor.
Tutoring Request for Sponsorship Student If approved, you will be notified by email.

Request for Master’s Exam or Portfolio Presentation Student, Faculty Advisor The Master’s Exam, Portfolio, and/or Thesis is the final requirement for many master’s degree programs and must be completed prior to your anticipated graduation date. See your faculty advisor for details and exam dates.
